Housing Case Manager

4 WEEKS Paid-Time Off!, Birthday Holiday PLUS MORE!
Shift: Monday-Friday: 10am-6pm or 11am-7pm
Assisting veterans with finding affordable, Safe and stable permanent housing in the community upon discharge from the Hope for Veterans Program.
- Provides non-clinical case management services including intake, assessment, housing stabilization, budgeting, financial entitlements/benefits, etc.
- Provide supportive counseling for veterans in need of crisis intervention and emergency services
- Support veterans in their recovery through psychoeducation groups, housing search and placement, and linkage to community-based services
- Develop Individual Recovery Plans (IRP) with veterans that adhere to VA Grant & Per Diem guidelines.
- Provides housing search services and support to all veterans with an upcoming discharge date.
- Provides advocacy, linkage, and referrals for permanent housing. May meet with veterans in the community in the course of service delivery.
- Acts as a mediator between veteran and landlords, realtors, real estate brokers, housing authority personnel, and any other housing professionals.
- Develops and nurtures relationships with landlords invested in supporting veterans. Liaison with other community and governmental agencies, landlords, real estate agents, and real estate brokers.
- Assists veterans with applications for housing subsidies and/or rental assistance.
- Assists veterans with coordinating move in and securing necessary household essentials through low cost connections.
- Function as a team member including emergency shift coverage, communicating all relevant information to other team members, covering co-workers’ duties as needed.
- Transports veterans in agency vehicles in a safe, cautious and responsible manner.
- Documents service delivery and maintains accurate, timely, and detailed records.
Qualifications:
- Master’s degree preferred in Social Work, Psychology or a related field; BA plus equivalent work or life experience will be strongly considered.
- 1-year of prior experience in housing services required, preferably in working with veterans.
- Prior experience in community outreach, marketing, or communication preferred.
- Must possess a valid driver’s license, with a good driving record.
