Do you have a desire to serve those who bravely served our country?&nbsp;If so,&nbsp;COME JOIN OUR TEAM!</span><br></h1> <p> </p> <p><span><b><font color="#000000">***4 WEEKS Paid-Time Off!, Birthday Holiday PLUS MORE!***</font></b></span></p> <p>Hybrid position (Will need to report to the main office in Parsippany occasionally for training, team meetings, equipment pick-up, etc.).<span><b><font color="#000000"></font></b></span><span></span></p> <ul><li><span>Providing case management services to Supportive Services for Veteran Families Program.</span>&nbsp;</li><li>Assist veterans in gaining entry to VA healthcare (including mental health care) or community care if veterans are not eligible for VA healthcare.</li><li>Assist veteran household members in gaining entry to healthcare (including mental healthcare).</li><li>Connect veterans to VA healthcare by working with the VA Medical Center (VAMC) to facilitate enrollment.</li><li>Help gather documentation and complete paperwork required for enrollment.</li><li>Follow up on enrollment progress to ensure that the veteran is enrolled in VA or community healthcare services.</li><li>Support veterans in identifying healthcare needs and barriers to adhering to recommended healthcare plans.</li><li>Work collaboratively with healthcare teams to facilitate access to care.</li><li>Assist veterans in understanding and communicating with providers to make informed decisions about healthcare.</li><li>Problem-solve barriers to care (i.e. transportation, childcare).</li><li>Assist veterans in utilizing available services including preventative healthcare.</li><li>Provide education or create linkages for veterans to learn about wellness related topics.</li><li>Documents service delivery accurately and timely while ensuring veteran confidentiality.&nbsp;</li></ul> <p>&nbsp;<strong>Qualifications:</strong><strong>&nbsp;</strong></p> <ul><li><span>Master’s Degree in Social Work, Psychology, Counseling, or a related field.</span></li><li>1-year of prior experience in social services or healthcare working with veterans or the homeless required.</li></ul> <p><em>VEVRAA Federal Contractor.
