AJT - Sr. Case Manager

SHIFT: Monday-Friday (Monday: 2pm-10pm, Tuesday-Friday: 8am-4pm)
Providing case management and support services to veterans enrolled in a residential veteran program. This position assists with the daily operation of the residence including residence safety, program coverage and staff scheduling, and emergency shift coverage.
- Assists in the development and implementation of recreational activities for veterans
- Assists Program Coordinator in transitioning new veterans to the residence and discharge or transition of outgoing veterans to other placements.
- Provides services including assessment, evaluation, recovery planning, case conferences, referrals for adjunctive services, family meetings, group work, individual counseling (as needed), veteran advocacy, linkage to community resources, and discharge planning.
- Develops recovery support groups and other activities to meet veteran needs.
- Performs on-site drug and alcohol screenings as needed.
- Provides medication education.
- Develops, supervises, and facilitates recreational and socialization activities for veterans.
- Convenes monthly veteran meetings to provide information to veterans and discuss all house related issues.
- Transports veterans in agency vehicles as needed.
- Documents service delivery, maintains accurate and timely clinical documentation.
- Ensures accuracy and consistency with the agencies fiscal and billing procedures.
- Ensures a safe, clean and adequate living environment for all veterans.
- Supervises and assists veterans in housekeeping, maintenance and upkeep of the facility.
- Maintains accurate and timely documentation including meeting minutes, fire drill records, chore lists, daily logs, and appropriate clinical documentation.
- Functions as a team member: covering shifts, facilitating groups, participating in program activities, communicating all relevant information to other team members, and covering co-workers duties as needed.
Qualifications:
- Bachelors Degree in health-related field.
- Minimum 1 year of case management experience, preferably in working with veterans, in a mental health setting or with individuals experiencing unstable housing or homelessness.
- Must possess a valid driver's license.
